Our care services

Our home operates on a 24 hour per day, 7 days per week staff supported services for our users, in a safe and secure enjoyable setting where young people can progress, with our end goal being for them to continue to live their life independently.

We help our service user develop or identify their interests, encourage enjoyment of learning, building their self-esteem and confidence and show them how to build meaningful relationships and achieve a successful transition to independence.

With our experience staff, who build effective professional communication and relationship with the young person. We endeavour to deliver high quality care and support to the young person by ensuring our staff accessing the necessary training to improve their knowledge and skills to implement a high standard of services and support to our services user.

• Each Young Person has an allocated Key Worker.
• YP Homes also provides follow-up support when the young person moves into their own accommodation.
• Low – Solo/Shared accommodation with medium support
• Medium – Solo/Shared accommodation with medium support
• High – Solo/Shared accommodation staffed 24/7 with high level support

Packages include:

• YP Homes offer Bespoke individually tailored care plans, Continual support through their education and career pathway, helping with CV writing, training courses, enrol in college and supporting with employment and links with job opportunities in the local community and register with GP practice and Dental Surgery.

1:1 key work sessions focused around safety, building positive relationships, maintaining personal hygiene, health, education/employment/training.

life skills including cooking, promoting healthy eating, Budgeting and basic house chores, supporting young people to become independence and be able to cope in society by showing the importance of maintaining well-being and self-care.
